This is the Paris Hilton CD as altered by acclaimed British street/guerilla artist Banksy. You can read about the prank in the BBC news item from September 3rd 2006:

500 of these were sneaked into UK music stores,
and it appears the majority have been discovered and removed by the
stores. I was fortunate enough to secure 2 two copies just a few hours
after the news broke, and I'm offering one of them here whilst keeping the other for my own collection.
The booklet is extensively modified from the original, as you
can see in the pictures here and in my flickr photoset (of one of the
actual CDs in my possession):

The photographs have proven very popular, which reflects the interest in Banksy, Paris
Hilton, and the prank itself. It's also interesting that mine are the
only pictures I've seen of the item anywhere, so I don't imagine there
are too many in private hands at this stage.
The CD is a plain white label with a hand-written "Paris" logo (marker pen). The audio is a 40 minute 'remix' featuring samples of Paris Hilton's voice throughout. The outer slipcase is unaltered from the original.
Any questions about this item please check my Flickr stream
comments or send me a message here and I'll try to answer them. Good
luck securing this - it's a nice item and a good way to get a very
limited Banksy piece in my opinion.
